Job Title: Self Employed Contract Support Worker
Reports to: Social Care Manager
Job Purpose
To provide support appropriate to the assessed needs of service users in order to enable them to gain the skills, knowledge and experience required to take control of their lives, and to engage with, and to live independently in the community. To facilitate this, support workers will be required to work in close partnership with the individual service user, primary and secondary care professionals, and others within the service user’s social network.
• Uniq Care and Support undertakes on-going assessments of all referrals
• Provide housing/social work related support packages to assist people referred to the service to learn Life and Social Skills.
• Co-operate with other agencies in identifying, planning and preparing service users for independent living.
• Offer advice and guidance with the aim to assist families/young people to establish a structured daily routine.
Main Responsibilities
1. To work in partnership with service users, their family members, team members, primary and secondary care professionals, to assist in the effective delivery service in order to provide the best outcomes for service users.
2. To carry out domestic cleaning duties on a daily basis.
3. To interact and communicate with service users in a way which promotes a positive and honest relationship.
4. To support service users to learn personal, domestic and social skills, in order to facilitate independent living.
5. To work in partnership with service users, and members of their family where appropriate to ensure that the rights of service users are respected, and professional and personal boundaries are maintained.
6. To support service users to learn skills that will enable them to access to further education, college courses, paid or unpaid work.
7. To ensure that support provided reflects the contents of the individual support plan.
8. To carry out record keeping and to write reports as required by the service.
9. To support service users to participate fully in the community, by working with them to develop a range of activities which reflect their individual and personal choices. To assist service users to develop and maintain a range of contacts and relationships in the community. For example GPs, and utility companies.
10. To carry out duties and responsibilities with due regard for the respect, dignity and privacy of service users.
11. To manage all information and communication that relates to service users, with sensitivity and respect for their confidentiality at all times. To manage all information into and out of Uniq Care and Support, in line with the organisation’s confidentiality policy.
12. To keep abreast of and maintain an up-to-date knowledge of legislation and benefits which pertains to service users
13. To assist and participate in the delivery of programs relevant to the identified needs of service users.
14. To assist the monitoring and evaluation of the services and its operations.
15. To carry out the above tasks in accordance with Uniq Care and Support Equal Opportunities Policies and Procedures, including its equal opportunity and diversity.
16. To carry out any other duties requested by the manager.
