SPECIALIST MENTAL HEALTH and DUAL DIAGNOSIS - REHABILITATION SUPPORT WORKER
Role: Rehabilitation Support Worker [Supported Living]
Responsible to: Registered Manager
Location: Hightrees Enfield
Key Role and Responsibilities:
To support, encourage and enable our service users to maintain or increase their level of independence. To provide practical and emotional support to assist service users with they’re Mental Health, medication and rehabilitation programme. To provide practical support to enable individual’s to maintain their tenancy and to offer support to maintain a safe, clean living environment. To nurture a homely atmosphere of inclusion and safety. Be proactive in supporting and encouraging service users to participate in the community activities, [college, work, social events] and identify personal goals, responsibilities and community mindfulness.
Main Duties:
• To work within company and legislative policy and procedures and to maintain Hightrees standards of care and support
• To report any and all concerns, complaints and compliments to the service management team
• To undertake regular and on-going training
• To liaise with medical professionals, law enforcement, family members and others on behalf of your key client
• To take on keyworker role and responsibilities, mentoring, budgeting, cooking skills,
• Monitor and/or administer medications
• To engage with and support our services users through the rehabilitation programme including the “highs and lows”
Administrative Responsibilities:
• Ability to follow an administrative and practical dual diagnosis programme©
• Ability to support individuals with benefit/job/educational applications and financial/budgeting responsibilities.
• Complete H and S and Environmental checks and to work in accordance with the company policies to ensure these and legislative requirements and standards are met
• To carry out key worker duties, arranging meetings, reviews, interviews,
• To maintain clear accurate records
• To have an understanding of the Care Quality Commission [CQC], how this body affects the work we do and the Key Lines of Enquiry [KLOE].
• To have an understanding of Safeguarding of Vulnerable Adults [Sova] and Deprivation of Liberty [DoL’s]
• To be willing to undertake training on the Mental Health Act and how this affects our service users

General/Other Responsibilities:
• To successfully complete Certificate in Care
• To undertake on-going and regular knowledge and skills training
• To maintain professionalism at all times as you will be representing Glenholme Mental Health Care Ltd.
• To undertake other duties that are commensurate with the post or that are agreed with the management team.
