We're looking for a nurse or ex-nurse to join our Homecare Team.  experience in community or end of life care is desirable but we would consider other backgrounds.
The role is office based with regular site visits out clients who use the service. The role includes:

Assessment of clients
Care Planning
Manage and deliver regular review program for clients.
Identify, manage and resolve risks to the service through regular review of clients and staff.
Act as a knowledge resource to the team.
Deliver local training and help develop learning plans for unregistered staff.
Provide support to the oncall service.
