POST: Field Supervisor
Hours: 35 hours per week
Responsible to: Designated Care Coordinator
Accountable to: Home Care Manager
Personal Specification:
experience of working in health and / or social care related field.
Evidence of some relevant training in health and / or social care [desirable]
Ability to demonstrate skills relevant to role [as per core skill requirements of the Agency].
Knowledge of issues around provision of care to vulnerable client groups.
Qualified to NVQ3 or QCF Level 3 Diploma in Health and Social Care
Ability to provide care to a multi racial client group
Ability to advocate for vulnerable clients
· Ability to work independently and use own initiative
Ability to communicate effectively [verbal and written]
Ability to contribute to care management process
DUTIES
To work alongside your designated Care coordinator/Branch Manager and undertake and agreed case load review, spot checks, and risk assessments as required,
To undertake all care and / or domestic duties in an emergency as advised at time of allocation / placement to prevent service failure.
To work in partnership with health and social care professionals in monitoring the implementation of agreed programs of care.
To liaise with relevant professionals or agencies in relation to client care or welfare e.g. Social services, housing departments, voluntary agencies.
To facilitate the independence of clients, in order to enable them maximize their potential through quality monitoring.
Work to meet own “Performance Targets” on a monthly basis
Working in partnership with clients and other relevant person(s) to develop and implement personalised programs of care.
Initiating and undertaking care reviews and all other care management duties
Fostering alliances and maintaining links with purchasers and clients to maintain partnership working and goodwill.
To undertake relevant training as advised by First Choice Homecare, Nursing and Recruitment in support of their role, whilst working
To respond swiftly and efficiently to staff absences or emergencies that may adversely impact on clients. Ensuring clients are informed promptly and measures are in place to minimise disruption to clients and services (including undertaking care duties in such instances if required)
Maintaining up to date records of all audit and monitoring activities and disseminating relevant information to all appropriate parties, alongside the Homecare Manager, as required
Work to meet own “Performance Targets” on a monthly basis
Monitor and alongside the Homecare Manager, co-ordinate all relevant Contract Monitoring and Contract compliance activities
Meeting with the Homecare Manager on a weekly basis (or as required basis) to discuss relevant operational issues including providing staff and client feedback
Identify own training needs in direct relation to the role and work in partnership with the manager to meet them
