Harley Street Care is looking for an experienced Care Coordinator to join our team.
We are looking to recruit an enthusiastic, dynamic individual to join our ever-expanding home-care agency. We operate in a unique, family-type manner with a tight-knit team who support each other, in a fast-paced yet friendly office environment. Our client base is focused around Central, North and West London, but your place of work will be Camden.
You will be dealing with clients as well as care staff on a daily basis, therefore, a compassionate approach is key to this job role.
Main purpose of job
To make sure customers receive excellent quality care and support to
help them remain safe and comfortable in their own homes. Compassion,
good communication and administrative skills together with a calm and
caring manner are vital for this important role in our company
Ensuring high standard of care through the supervision, assessment , and training of care staff and the monitoring of clients’ changing needs
To build and maintain positive relationships with service users and
their families, through frequent interaction
Make sure care assistants are appropriately allocated to customers so that care is delivered on time, safely and in line with customers wishes (as agreed in their care and support plan).
Key tasks/main duties
Be the main point of contact for new customer inquiries when in the office.
Assess and contribute to the development of care plans to provide
safe, effective care and support that responds to each customers’
needs and preferences.
Support each customer to live their life the way that they choose as far as they are able. Respect customers’ choices and promote their dignity and privacy at all times.
Review effectiveness of care plans and allocated staff and amend as required.
Carry out a Risk Assessment. Agree on appropriate risk control measures to reduce identified risks. Keep care and support plans up to date and escalate concerns without delay.
Provide the customer and, where appropriate, their representatives with information about the service so that they are clear about what to expect and how they can raise any concerns.
Be the main point of contact for all customers, family and health professionals on an on-going basis when it comes to the service we provide.
Keep all information about customers and their families secure and confidential except where policy requires you to share to protect the interests of our customers.
Liaise with the care manager to make sure sufficient care assistants with the right skills mix are recruited to meet the needs of the business. Arrange cover for care assistant sickness, absenteeism or holidays.
Accept, allocate and process new referrals for care and support promptly.
Monitor the allocation of care assistants to maximize efficiency
whilst supporting them to maintain an appropriate work/ life balance.
Distribute staff rotas weekly.
Work with the office team to maintain up to date electronic and hand
written records. Make sure that accidents and incidents are recorded,
reported and acted upon.
Carry out general office duties and prepare reports as required as required by the care manager.
Skills/Qualifications:
Must have Care-coordinating experience.
NVQ Level 3
Previous experience in a care agency or senior carer role would be advantageous.
