Case Manager (£40,000 per annum plus benefits)

Role profile:
We have exciting opportunities for strong non-clinical Case Managers to manage our complex healthcare and social support services to patients in the community.
You will be managing the co-ordination of our ongoing care packages. You will work with patients, CCGs, MDT and other providers to ensure our care packages are always planned ahead and managed robustly. You will be responsible for driving high levels of service quality and patient satisfaction through building strong relationships with our patients, CCGs and colleagues and taking a proactive approach.
Working with internal teams to deliver excellent and empathetic customer services through training and excellent management. Your objective will be to prevent service or quality issues, proactive management to resolve issues when they occur, liaising with the relevant internal/external teams.
experience required:
You will be a skilled case manager with experience within the healthcare or ancillary care sector. You will be passionate about delivering exceptional services for patients. Building relationships and managing stakeholders is second nature to you and you use these skills to enhance the offering for your customer and resolve issues and problems.

Ø Full driving licence
In return you will receive:
· Pay: £40,000 per annum
· Hours: 37.5 per week based in Wimbledon with some travel
· Annual leave: 25 days plus bank holidays
· Fully expensed work travel
· Laptop and Mobile Phone
