- Assess the residents physical, psychological and social needs and compile a care plan in collaboration with the resident, family and any other relevant health care professional
- Plan, implement and evaluate the nursing care in relation to the residents dynamic needs in accordance with clinical policy and regulatory framework with other appropriate health care professionals
- Ensure all the entries into nursing records meet NMC standards for record keeping
- Participate in the formal audit programme within your level of competence
- Maintain registration with the NMC
- Comply with the NMC code at all times
- Allocate an appropriate key worker to each resident on admission
- Visit each resident each day whilst on duty and ensure appropriate nursing care is delivered in a professional and timely manner by the care team under your supervision
- Support the senior clinical staff in the home and if appropriate and within your level of competence assist with any clinical investigations or complaints
- Comply with the NMC administration of medication administration and clinical policy to ensure that processes are followed to ensure the safe ordering, custody, storing, disposal, recording and administration of all medication
- Manage stock levels and replenishment of all appropriate clinical supplies
- Maintain and develop continual professional development and clinical skills to enable safe and effective clinical practice in line with NMC revalidation process and contribute to continuous process improvements with regard to clinical and non-clinical policy
-Maintain professional communication with clinical line manager and take every opportunity to discuss the quality of care and any clinical concerns that may arise and any issues related to safeguarding in a timely manner
- Support residents in understanding the ageing process and how it affects their health and lifestyle. Support residents to maximize their independence and create the environment which supports their physical and mental well-being and ensure that all residents are treated with dignity
- Adhere to the local safeguarding policy and the reporting mechanisms. Contribute to any safeguarding investigation within the home, following the Duty of Candour requirements
- Comply with the accident/incident reporting policy to maintain the safety of staff and residents in the home, following the Duty of Candour requirements
- Support all residents with their individual diagnosis who may require additional psychological or emotional support
- To model person centered care in your practice
- Lead a multi-disciplinary shift team, supervising staff in all respects including orientation of new and agency staff, allocating and coordinating team members to deliver care to the appropriate resident groups and supervising the delivery of quality care delivered to residents providing support, supervision and mentoring as required within your scope of competence and as per the NMC code
- Assist in the recruitment of care staff in the home, including the development of clinical skills, supervision and induction programs as well as mentoring as per the NMC code
- Work collaboratively with the activities team in the home to provide a comprehensive daily activities programme to ensure residents spiritual and social needs are met
- Support other clinical staff in the home with the care practitioner programme and contribute to the supervision, mentoring and competency of the care practitioner within your scope of competence
- To be key member of the Care and Quality team providing professional input into clinical projects and developments within the home
- To work collaboratively with colleagues across all disciplines within Barchester Health Care and external agencies to develop and sustain best clinical practice
- Be professional, courteous and transparent in your communication
- Develop close working relationships with other clinical staff in the team including the Care Home Clinical lead
- Any other reasonable task requested by your line manager or the care home Clinical lead or home management team which is within your competence
- Support the clinical training programme in the home through facilitating staff to attend informal and formal training opportunities
- Assist in the investigation of clinical complaints
- To practice within the Care and Quality Nursing strategies within Barchester Health Care.
